Although management makes every effort to accommodate individual preferences, business
needs may at times make the following conditions mandatory: overtime, shift work, a
rotating work schedule or, a work schedule other than Monday through Friday. I understand
and accept these as conditions of my continuing employment.

and may be required to submit to a physical examination. Refusal to take these tests will
eliminate my application for consideration or terminate my employment.
I further understand that this is an application for at will employment and that no employment
contract is being offered. I understand that if I am hired by GHA, my employment and
compensation can be terminated at any time with or without cause and with or without notice, at
the option of either the employer or myself. Further, I understand that if I am employed, such
